A. REVIEW OF THE FACULTY MEMBER'S PERFORMANCE


Enter the number of yes/no votes from the committee in consideration of the faculty member's overall performance. This voting process should happen outside of Digital Measures. For assistance with conducting the voting process, reach out to UTHSC Faculty Affairs by emailing [afsa@uthsc.edu](mailto:afsa@uthsc.edu).

Following the votes, enter comments regarding the conclusions, supporting reasons, and any dissenting explanations from the committee. Additionally, provide recommendations for the faculty member.

Dissenting explanation: For any conclusion about the faculty member’s performance that is not adopted unanimously, if a dissenting member chooses to provide one

B. REVIEW OF THE ANNUAL PERFORMANCE AND PLANNING REVIEWS (APPR)

Similar to the voting process completed in Section A., conduct a vote regarding the faculty member's Annual Performance and Planning Reviews (APPR). Enter the number of yes/no votes in the spaces provided.

Following the votes, enter comments regarding the conclusions, supporting reasons, and any dissenting explanations from the committee. Additionally, provide recommendations for the department chair/division chief.
