Accommodations Tracking in Data Collection Hub

From the filter view, click on your initials.

Click "Settings"

Click "Data Hub Forms"

Click "Create New Form"

Title the form and click "Add a Field"

Click any field that is needed (i.e. Date Field)

For accommodations, Click "Smart Accommodation Field"

Enter the tracking options (i.e. Implemented, Refused, Not Needed, etc.) Click the "Required Field" box.

Click "Save" when complete.

To begin tracking, there are two ways to do this. One is to click on the student's profile.

Click "Data Hub Submissions"

Find the Smart Accommodation Tracking Form module and click "Add"

Click the "Enter Date..." field and continue to complete the form.

Click "Submit" when finished.

You will then be able to view the table for the form you just filled out.

Another way to complete the form is to Click "Data Collection Hub"

Click on the "Smart Accommodation Tracking Form"

Choose the students you are tracking accommodations for. (Multiple students can be chosen.)

Complete the tracking by clicking on the student's name and viewing their current accommodations. Click "Submit" when finished.
