Action Plans: Creating a Document Upload Activity

Part 1: Creating an Activity in Admin. View

To create an activity, you need to be in the Admin view of the LMS. To get started, select **Settings**.

From the **Settings** drop-down menu, select **Switch to Admin View**.

Go to the **Activities** tab and select **Activities**.

Navigate to the **Manage Activities** tab and select **Manage Activities**.

Next, select **Add Activity**.

Next, click **Attestation**—these are activities where completion depends on the Learner, an group admin or both signing off that the required activity has been completed.

Next, start filling out the Attestation template. Click the **Title** text field and enter a title.

Next, enter a description for the activity

Optional: Click "Choose file" to add a visual to the activity.

Scroll to the bottom of the template and select "Next".

On the **Learner Attestation Required** page, click the Sign Off Instructions text field to add further instructions to define expectations.

**Note:** Please select the checkbox located below **Evidence**.

Next, choose the evidence type: **Document Upload, Text Field, or Audio Recording**. **Note:** Only one evidence type can be select for an activity.

Then, click **Add Content**.

Fill out the content template and upload a file for users to access.

Then select **+Add button**.

Once the content template is no longer visible, click **Apply**.

The page will redirect to the **Activities** tab.

If the title you created is not visible, enter the activity title in the search field. Once it appears, click the **Settings** drop-down aligned with the activity.

Select **Activate** to make the activity visible in the Action Plans template.

Part 2: Adding Created Activities to an Action Plan

To access the Action Plans area of the LMS, you’ll need to exit Admin view by clicking **Account Settings**.

From the drop-down menu, select **Switch to Learner View**.

Once redirected to the main dashboard, click **Settings** and select **Action Plans**.

Click the **Create a Plan** button.

Complete the action plan template.

Scroll to the bottom of the template and select **Next**.

The action plan structure will appear. Locate the folder icon and right-click on it.

Hover over **+ Add** and select **Activity**.

Click the **Type to search** field and enter the title of the **New Activity** you created.

Once the activity appears, locate and click the **+ Add** button.

Next, review the action plan structure to confirm that the activity has been added.

Then, scroll to the bottom of the page and click **Apply** to save and upload your action plan.

Lastly, the page will redirect you to the Action Plans area.
