Add your question details into the "Title" field

**Question do not automatically number** - if you want your questions to be numbered, please add it to the title

You will then be presented with 6 sections of the question build to update

 Question Details
 Theme Groups
 Restrict Organisation Types
 Answer Capture Options
 Specific Rating Explanations (if applicable)
 Matrix Options (if applicable)

Here you can update the "Section" and "Order" where the question will sit within the audit

"Summary" and "Description" boxes can be used to add any additional information required to your question, including media, images etc.

**Restrict Organisation Types -** If the question is not suitable to one of the organisation types, you can use the check-boxes, to choose which Org Types it is hidden from. In the example shown above, that question will be hidden from all organisations in the Buckinghamshire - Early Years group

**Answer Capture Options -** you will now need to select the your answer type(s) required for your question, for more detailed info on the answer options, please see [Which Answer Capture Options is Right For You?](https://academy.phew.org.uk/en/knowledge/which-answer-capture-option-is-right-for-you)

If creating a **Ratings** question, and you need to add specific explanations of each rating option, add them here

If creating a **Matrix** question, click "Matrix Options" to build (for further information, please see [How to Create a Matrix Heading](https://academy.phew.org.uk/en/knowledge/how-matrix-headings-work)

Click "Save Question" to save your changes.

All questions are editable up until the point where the audit has been sent out to respondents.
