Assessments: Admin Rootwurks LXP Onboarding

This article walks staff users through the process of completing assessments assigned within the Rootwurks platform. The course also covers how to review your responses and what to expect if any follow-up actions, such as corrective action plans, are required.

How do I start an assigned assessment?

Logg into the Rootwurks dashboard

To start an assessment from your dashboard, begin by looking at the left-hand side. If any assessments have been completed in the past 30 days, you’ll see a summary card labeled “Assessments.” In the top right corner of that card, there’s a small green square icon—clicking this icon will launch the assessments section and allow you to dive straight into the assigned content.

As you move across to the right side of your screen, you’ll see the “Your Schedule” panel. This section helps you stay on top of deadlines with views by day, week, or month. Assignments appear grouped under headings like “Overdue” or “Others,” and if there’s an assessment due, you’ll see it listed by name along with its seat time and due date. To get started, just click the “Start” or “Resume” button next to the assignment. This is a convenient way to begin or pick up right where you left off—all without leaving your dashboard.

You can also navigate to the assessments section by clicking the hamburger icon in the top-left corner of your screen. This opens the main navigation panel. From here, scroll down to the “Evaluations” section and click on “Assessments.” This takes you directly to the assessments dashboard, where you can view everything that’s been assigned to you—whether it’s upcoming, in progress, overdue, or already completed.

From the assessments section, look for the assessment you want to begin. You’ll see key details like its name, due date, and current status. To start the assessment, click the **three-dot action menu** on the far right of the row, then select **“Start Assessment.”**

After starting your assessment, the first step is to choose the facility where the assessment will be conducted. If your organization has more than one location, you’ll see a dropdown menu that lists all available facilities. You can scroll through the list or use the search bar to quickly find the one you need. Once you select a facility, the “Start Assessment” button will become active.

Next, you'll see the summary of the assessment you selected. After reviewing the details, click the green **Continue** button to move on to the next step.

As you begin answering the questions in your assessment, make sure to read each one carefully and evaluate it against the actual conditions and procedures in your facility. Each question focuses on specific responsibilities or procedures and it’s your job to evaluate whether those standards are being met. You’ll respond to each item by selecting either “Yes” or “No,” depending on whether your facility or the employee you’re evaluating meets the stated criteria.

If you answer “No” to any question, the system will require you to either review and resolve the issue immediately or create a corrective action plan. This ensures that every identified gap is acknowledged and addressed.

If you need more flexibility while answering, click the three-dot action menu to the right of any question. This menu gives you several options: you can reset your answer if you want to change your response, mark the question as not applicable if it doesn’t apply to your facility, mark it as not assessed if you’re unable to evaluate it at the moment, or choose to “Add an Observation”.

When completing your assessment, you may also need to add observations to support your answers. Use the three-dot menu next to any question to open the “Add Observation” window. Here, you can briefly describe what you’ve observed and indicate whether the observation is positive or negative. Positive observations highlight what’s working well, while negative ones point out issues or areas that need attention. If you mark an observation as negative, you’ll see a checkbox labeled “Require Action Plan.” If this box is checked, the system will require you to either review and resolve the issue or create a corrective action plan. If it’s left unchecked, the observation will still appear in the final report but won’t trigger any follow-up action.

To add a note during an assessment, click the Notes icon located below the question. A yellow comment box will appear where you can type your note. Once entered, the note will be saved with a timestamp and the name of the person who added it. You can delete the note by clicking the delete button.

To add photos to a response in an assessment, click the photo icon beneath the question. This opens a panel where you can upload images from your device, such as relevant product or procedure photos. Once uploaded, the image will display below the question.

To attach a file, click the paperclip icon next to the photo option. You can then upload documents like SOPs, or other supporting materials.

When you’ve completed all questions in the assessment, you have two options at the bottom: **Review** or **Save & Exit**. Choosing **Review** moves you to the next step to review answers before submitting. Selecting **Save & Exit** allows you to leave the assessment and come back to finish it later without losing your work.

To finish your assessment, you'll be guided to the final step: **Review & Submit**. This screen provides a clear summary of all your responses, including selected answers, uploaded attachments, and any notes or observations added during the process. It's your opportunity to scroll through and verify the accuracy and completeness of your input before submitting. If everything looks correct and no changes are needed, click **Complete Assessment** to submit your responses.

How do I start an unscheduled assessment?

You might want to start an unscheduled assessment to follow up on a corrective action, verify changes after a recent issue, or prepare for an upcoming audit. To begin an unscheduled assessment, first open the main menu by selecting the icon with three horizontal lines in the top left corner. From the menu, go to **“available content”**.

Then click on the **Assessments** tab at the top of the page. You’ll see a list of assessments under either “Custom Assessments” or “Enterprise Assessments.” To begin one immediately, find the desired assessment, click the three-dot action menu on the right side, and choose **Start Assessment** from the dropdown.

How do I review the results of my assessment?

Once an assessment is completed, the results are immediately available for review. The summary screen displays the score, key details like who completed it and when, and any questions or observations that require action plans.

To review the answers, click into each section of the assessment. This expands the group of questions and displays your selected responses, any notes, and attachments. It's a quick way to verify what was submitted and spot any areas that need attention.

If any answers or observations require follow-up, they’ll appear here in the action plan summary. You’ll see them grouped by observations and by individual questions. To address one, click **“Create”** next to the item. This opens a form where you can outline a corrective plan or assign it to the right person.

You can also return to the results later. From the Management Tools menu, choose **Assessments** under the **Evaluations** section. In the “My Recent Assessments” list, click the three-dot menu next to the assessment you want to review and select **View Result**.
