Navigate to the State Reporting tab of the 120Water PWS Portal.

Click "Complete Customer Notice Certification Form"

Complete Customer Notice Certification Form

Toggle the method used to distribute your notices.

Review each notice type and mark if you distributed them or not.

For the categories of notices you did send, populate how many and the date sent. If you sent on multiple dates, input the last date notices were sent.

Upload the Certification of Customer Notice Form (required in GA) if applicable. For all states, upload examples of the notices that were sent for the state to review.

Toggle the certification and hit the "Submit Customer Notice Certification" button.
