This guide provides a straightforward process for healthcare providers to connect a patient's Freestyle Libre continuous glucose monitor (CGM) to their electronic medical record (EMR) enabling real-time access for providers to the patient's glucose data. 01/01/2026

Access the patient's dashboard from the EMR. Click the blue "Connect Devices" link.

Click the yellow "Connect Freestyle Libre Account" button.

First, you will need to log in as the patient by following these steps: 1. Enter the patient's email used to set up their Libre account in the "Email" textbox. 2. Enter the patient's password for their Libre account in the "Password" textbox. 3. Click the blue "Sign in" button.

Next, complete the HIPAA Authorization by following these steps: 1. Click the blue "Allow" button. 2. Click the blue "Agree" button.

Lastly, approve data sharing with endo.digital by following these steps: 1. Click the "Share the following data with Endo.Digital" toggle. It will turn blue. 2. Click the blue "Allow" button.

- The patient's Libre has been successfully connected. Click the blue "Ok" button.

There are two ways to confirm the Libre device is connected: 1. The yellow "Connect Your Freestyle Libre" button disappears, on the Connect Devices page. 2. The "Connected CGM device" information on the patient dashboard shows "Libre."
