To add the form addon go to your product -&gt; Product Addons and then select the addon form

To add pricing rules for a specific product variation do the following steps

Purchase x Receive y
Purchase x Receive y - same products
Exclude products from all rules

Select the "Single product variation" option.

Set the minimum quantity for that condition

Set the maximum quantity for that condition

Percentage discount

Fixed price
Percentage fee
Price fee

Click "Add" to add a new pricing condition

Click "+ OR New Group" to select the variation on which this pricing rule will apply
