Click the "Ask Client if this is their first time booking this treatment?" field.

Click the "Use 'Subsequent Treatment Consent Form' template in booking confirmation email?" box. If you select this option the 'Subsequent Treatment Consent Form' will be sent to the client in the booking confirmation email instead of the linked form if it is NOT their first time booking the treatment. If it IS their first time booking the treatment they will receive the linked form.
