Create a Journal Entry
20 steps
1
You can create a journal entry from three places: The Dashboard, an appointment, or the patient's Patient360.\ \ Journal entries created from an appointment or the Dashboard are automatically linked to that appointment. Journal entries created from Patient360 need to be linked manually afterward.
2
From the Dashboard, find the appointment and click "Add journal".
3
If you're creating the entry from an appointment, open the appointment and click "Create".
4
To create the entry from the patient's Patient360, click Journal, then click Create to start a new entry.
5
Either way, you'll have the same options to document it. The plain text field lets you write the journal entry as free-form text.
6
You can also use a plain text template that will give you a ready-made starting layout. The templates need to have been uploaded in advance.
7
For more advanced entries or a more designed layout, use Advanced Forms. This provides you with things like drop-down lists, multiple-choice options or drawings.
8
Choose the template you want to use from the pre-uploaded list and then Click "Use Template" to add it to the journal entry.
9
Complete the form using the available fields, such as drop-down menus, radio buttons, and drawings.
10
Once you're ready, click Submit. You can still edit your answers at any point up until the full journal entry is published.
11
Click the X to close the template and return to the journal entry.
12
Next, add a title for the entry in the Title field.
13
If needed, add attachments, images, products, implants, or medications to complete the documentation.
14
You can choose "Save as draft" and return later to complete and publish the entry.\ \ **Note:** Draft journal entries are not visible to the patient. If a draft isn’t published manually, it will be automatically published after 2 weeks and then become visible to the patient.
15
When everything is ready, click **Complete and Publish**.
16
After finishing, the journal entry appears in the patient's Patient360. If the entry was created from an appointment or the Dashboard, it will already be linked to that appointment.
17

18
The completed Advanced Journal also appears under Patient Files.
