The first step to creating a mid-term survey is to create a question set.

To begin the process of creating a new question survey, click on **Surveys & Projects** in the upper toolbar. Then, click **Surveys**.

To create a new survey, click on the **Create New Survey** button on the left-hand side of the page. A “Create New Survey” pop-up will appear. Click the **Begin** button.

Add a title to the survey (and add a description if you choose). Click **Save New** to advance to the next step.

A new dropdown field will appear, allowing you to insert labels or questions.

Add questions by choosing a question type and then selecting “Add.” In the box labeled **Question Text**, type your question. For questions with choices, you must also add **Response Options**, which is where you will put your scale.

Press the **Save** button to save questions as you create them.

Once you are finished creating the survey to your liking, click the **Save** button in the **Survey Properties** box.

The next step to creating a mid-term survey is to create a project to use the questions set you just created.

To begin the process of creating a project, locate **Surveys & Projects** in the upper toolbar. Then, click **Projects**.

This will take you to the Create and Manage Projects page. Here, you can click on **Create New Project** to begin the project creation process.

In the window that pops up, click on **General Survey Project**, and then select **Begin**.

Enter a project **Title**. You may want to include the term and course name in the title so you can  easily find it again.\

Next, input Project **Start and End Dates/Times**. This determines when the general survey period will start and end within the project.

In the box asking how many respondents you want to allow, you must enter a number above 0. It is better to over-estimate.

Now, we will want to attach questions to this project. Under the **Survey** tab, click **Select a Survey**.

A pop-up will appear with your existing survey sets. Select the one you would like to use for this project.

In the “Confirm Selected Survey” dropdown that will appear, select **Save Survey**.

To make the project accessible to students, we must now deploy the project.

In the **Deploy Project** tab, the dates you previously entered, as well as the number of respondents you indicated to allow, will appear.

If everything looks correct, you are ready to deploy your survey. Click the **Deploy** button, and select **OK** in the verification dropdown box.

You will know the survey has been successfully deployed if you see a green bar saying “Project has been deployed.”

To send the survey to your students, you will find survey links and a survey QR code under the **Properties** tab.

You can copy the links by highlighting the text with your mouse, right-clicking, and selecting “Copy.” You can Copy or Download the QR code with the respective buttons below the code.
