Select the desired Review Frequency -  How often does this compliance need to be submitted? 

When approving this compliance, do you need to submit evidence of the approval?

When rejecting this compliance, what action is required?

Click this checkbox if this compliance is required in more than one state. 

Click "Custom Forms" to locate your compliance.

Click "My New Compliance" to add questions to this compliance.

Click "Add Form" to name the first section of this compliance

Select a question type from our drop-down.

Toggle these options if you would like a file/comment to be mandatory when an option is selected.

Click this checkbox to mark this question as a mandatory requirement.

Click "Add Question Below" to add more questions.

Once you have created all questions, Click 'Edit' button to structure the format of your form.

Use this drop-down to add directions  and skip questions as needed. 

Click 'Save' once you have made your changes for each question.

Perform this for all of the questions you have added and when you reach the last question, select 'End the Form' as the next direction.

When you have created all questions, click "Start Trial" to test it out!

Click "Publish" to finalise the form you have created.

Click "Location Requirements" to configure this compliance to a location.

Select your compliance from the drop-down, complete all required fields and submit!
