Choose your preferred form type on the 'New Forms' page. The available form types include General, Compliance Consent, and Account Engagement.

Enter the form name in the "Forms Name" field.

Select the "Active" box if you want the form to be visible to logged-in Scan app users. Otherwise, leave it unchecked.

After saving, you'll be taken to the form's page. Here, you can view and edit the form's details under the "Details" tab.
