Navigate to [https://login.myocchealthrecord.com](https://login.myocchealthrecord.com/)

Start off by clicking onto the Employee Health Management module

Search for the name of the employee you are uploading records for and select the blue icon button next to their name

In the employee's profile, click the Health Surveillance tab and scroll down to the Health Surveillance window. Click the "upload record" button.

Please complete the required fields only, marked with an \*. Select the activity type related to these results.

Select the "Activity Date" that these results were taken.

Select the "Rating System" and "Activity Status." These will appear in the Health Surveillance window allowing users with permission to view these results, to quickly review the status. This is shown in step 14 of this guide.

Tip! If you need to create a reminder based off this record, you can do so by filling in the reminder date and reason fields. This will create a reminder for this employee, which can be found in their reminders window.

Alert! If you are uploading sensitive data, ensure you select "Full report" or "Medical component," so that users without permissions to view full employee data cannot access these uploads.

Next, upload the document containing the results.

The uploaded document will appear below. Once confirmed, click "Upload."

Once uploaded, the new record will show in the Health Surveillance table.

If you added a description, the information can be seen here by hovering over the information icon under "Description" in the table.

Next to the Description column, you can find the Activity Status.
