Navigate to <https://login.myocchealthrecord.com/>

To activate a reminder, start off by clicking on the "Employee Health Management" module and searching for the employee's name. Once found click on their profile. A new tab will open in your browser of the employee's profile, click onto the "Health Surveillance" tab and view any reminders set.

Select the desired health provider from the first drop-down and the assessment that's required to be completed for this reminder from the second.

As you are booking an active reminder, the toggle to indicate this will automatically be set to "yes."

Tip! If this reminder is either sensitive or high priority, you can tag this booking request and your provider will be notified.

Search for by the suburb or postcode closest to the employee so the provider can arrange their appointment to the closest clinic.

If you are aware of dates and times this employee is available, you can add them in the below fields.

You can leave any notes related to the employee in the below notes field, this information will be accessible for your provider to read when reviewing the booking details. Once all information is confirmed, click "Create."

The reminder has now been activated. To check the progress of this booking request move into the Health Surveillance module, then click into the "Booking and Appointment tracking" tab to search for the employee's name and view the progress.

Tip! To view the in progress reminder from an employee's EHM profile, click on the Health Surveillance tab and find the "Health Surveillance reminders" window. Hovering over the status will provide you with a reminder history.
