If the business is a single-member LLC (or disregarded entity for tax purposes), you must put your name on Line 1, and put your LLC or DBA name on Line 2.

For instance, if you are the only owner of Uncle Sam Hats LLC, you would write your full legal name on Line 1, and you would write "Uncle Sam Hats LLC" on Line 2.

If you are a multi-member LLC, reporting taxes as a partnership, S-corporation, or C-corporation, put your business name on Line 1.

Fill in Line 2 if applicable. See "Note" above for guidance.

In Line 3, check the box that matches your business class.

Skip Line 3b and Line 4 unless applicable to your business.

Enter your business address in Line 5 and 6.

Note: Don't forget to include apartment or suite numbers

In Part I, enter your Taxpayer Identification Number (TIN) - this will either be your SSN or your business' EIN.

Note:

If you’re a single-member LLC that has not elected S-Corp or C-Corp status, use your SSN or EIN if you have one.

If the single-member LLC has made an election to be taxed as a different business entity type, use that entity’s EIN.

If you’re a resident alien rather than a U.S. Citizen, use your **Individual Taxpayer Identification Number (ITIN).**

In Part II, add your signature and today's date.

Note: Add your signature and the date by 1. Printing out the W9 and signing it physically or 2. Using a digital editing software such as Adobe Acrobat or a PDF editor.

Upload your W9 to the secure submission portal. If you printed out your W9, take a photo or scan the page to upload the signed version.
