Click this fix icon to identify the problem.

Click configure on the checkbox and radio missing fields.

Click the "Enter option name" field that is blank and add an option, even if it is just "yes" or "I agree". 

Click "Save and close". (You may have to make a small change to the page, like adding a space to the end of text before it lets you save and close). 

After a minute and a couple refreshes you should see the "Fix" change to "Accessible!"
