We will automatically calculate the amount of overtime completed based on your working pattern and any other leave.  You can edit this and it will then be flagged for a manager to check. 

The hours worked in this overtime based upon your working pattern. This does not consider breaks - please check that breaks have been correctly deducted.

Choose whether you'd like this as Time Off In Lieu (TOIL) if your practice offers this option.  Ensure you are within the TOIL criteria requested by your practice. 

Choose the rate of pay for this overtime. This is a default of single time, other agreed pay rates will be shown. You should ensure that you add this as agreed with your practice.

Time and a quarter
Time and a half
Double time

You can add any additional relevant information in the "Request Notes" field.
