Search for the worker's name and click on their name.

Tip! If the worker cannot be found, you can create a new profile for them. Fill in the required fields and click "add".

Now you need to provide the provider and assessment type you need to request for this worker.

If you only have 1 Provider this field will auto populate with their name and you will not need to select anything. If you have multiple providers click the dropdown menu to view them all.

Click 'Available assessment' to view the assessment/s linked to this org unit and the job type of the worker. If only 1 assessment is linked, then it will auto populate for you.

If a medical is related to an active reminder in this workers EHM profile, click here to link it.

Tip! To tag an assessment that either is sensitive or high priority click one of the below options. The provider will be informed of this information.

All the information provided in this final booking step is passed securely through to the provider to help them facilitate the most convenient appointment for the worker.

Select the workers availability by adding in their preferred dates and times they can complete their assessment.

If you need to leave any notes regarding this worker's request to the Provider's booking team, please leave them here.

Once you have reviewed the request, click "Create".

When you have successfully created a request, the below message will pop up.
