Navigate to [https://portal.geoh.app](https://portal.geoh.app/dashboard/work-list?category=visit_check_in_missed)

Click the name of the client who needs the care plan

Click the "Visit Type" field and add the visit type associated with the care plan

Click the "Default Tasks" field. A dropdown will appear. Select the tasks you'd like associated with this care plan, or you can click "Select All" to add all tasks associated with the visit type

If required for the client, put diagnosis code here
