Navigate to [https://portal.geoh.app/](https://portal.geoh.app/dashboard/work-list?category=visit_check_in_missed)

Enter the client's address.  After typing in a portion of the address, the system will try to auto complete it.  Please select the correct option when it shows up.

You can click "Save" now to complete the process, or continue to add a new care plan for the client, which will allow you to add diagnosis codes as required by some Notices of Admission.
