How to Request a New/Replacement Lab Caution Sign

This guide provides a straightforward process for submitting a new lab caution sign request through the Oregon State University EHS portal. Follow these steps to efficiently provide necessary contact and safety information for your laboratory facility.

From the EH&S Homepage <https://ehs.oregonstate.edu/>Click "Request a Lab Caution Sign" on the "How Do I" drop down menu.

Click "Request a Sign"

Mark off boxes as appropriate for your lab hazards. These are the icons that will appear on the sign. Scroll down and mark off all applicable options.

At the bottom mark off all equipment that is in the space from the list. Click the forward arrow when its complete.

Fill out the PI info, as well as info for any lab managers or other contacts that should be on the sign, including the department info at the bottom.

Click on the forward arrow when the request is ready to submit.

Your sign request has now been submitted. You will receive an email from Qualtrics confirming submission. EH&S will create the sign and post it at your requested location. If there are any questions about the sign contents, EH&S will reach out to confirm any needed information or ask for details.
