Navigate to [https://www.cbizesc.com/lssd/](https://www.cbizesc.com/lssd/)

Enter your username and PIN and click "Login".
Follow the onscreen instructions to determine your username and PIN.

Click the checkbox for "I accept" to enter the enrollment site.

Click "here" to view and update your beneficiary designation(s).

Click "here" after reviewing the Enrollment Tips. 

Enter the email address where you would like to receive your confirmation statement, then click "Next >>"

Review the instructions and then click "Next >>".

If you would like to see your current beneficiary designation (and covered dependents), click "Review"

Click "Return" to go back to the previous page to make updates. 

Click on the dependent/beneficiary name to edit them. 
Please note the following tips: 
1. You cannot delete a beneficiary/dependent, but you can mark "No" to all beneficiary types and save. 
2. In order to select a new Primary or Contingent beneficiary, you may first have to mark "No" on the previous beneficiary. 
3. When splitting your life proceeds between more than one beneficiary, the total for each beneficiary type must be 100%. 

Make your edits as needed and then click "Save".

Once all updates are made, click this checkbox to confirm all information is accurate.

Scroll down to the bottom of the confirmation to see your Beneficiary Designations. 
If something does not look correct, click "Change". This will take you through the process again, starting at the email address. 

If everything is correct and you want to keep a paper record of your change, click "Print".

If everything looks correct, in order for your change to be submitted, you must click "Confirm". 
When you click "Confirm, it will also send you an email to the address you entered with a copy of this confirmation statement.
