Choose the Protocol you needed from the Drop Down, here we select 
TCP & UDP for FiveM Proxy

Click on origin and input your FiveM server IP here.

Click on origin Port and input your FiveM server Port here.

Leave everything as default and Click Save.

You will see a message like this, after a successful creation.

Click on the new Protect Proxy and copy it.

Tip!
• Make sure your Server and host firewall is Off. (unless you know how to pass the proxy ip:port through firewalls.)
• Make sure you are using a new public ip that is not exposed to public.
