From the MMA website, click on the hamburger

Click on AT Travel Waiver Form - Then Fill out the form on the right.

Make sure to fill in your age and Coach's Name and Email along with your sport. If you are under 18 please provide a Parent/Guardian email. If your Parent/Guardian doesn't get an email please have them check their junk/spam folder for an email from softdocs@maritime.edu. Optional: select a second sport if you play more than one.
