Select the client you would like to send an online form to via your clients' admin module.

Once in the timeline of your client's medical records, please select the online forms icon.

Online forms allow you to request/collect in advance of care information such as screening tools, intake forms, demographic information, and consent for care from your client via email or text. *Note that you need to activate online services before you can use this feature. Once you send the form(s), your client will be able to complete the information requested and send it back to you. The information will be collected directly in the client's timeline and you will be notified once the form has been sent by the client for review.
