How do I complete an internal audit?

Navigate to Rootwurks Dashboard

There are a number of ways to launch an internal audit in Rootwurks. From the dashboard, locate the Internal Audits card and click the open icon in the upper right corner which will take you to the internal audits page. You’ll see a calendar with due dates and a list of audits assigned to each facility. Simply select the audit you'd like to work on to get started.

You can also access internal audits through the main navigation. Click the hamburger icon in the top left, then select “Internal Audits” under the Evaluations section. From here, you'll see your audit schedule and any overdue items. Click on an audit name to open it.

As you move across to the right side of your screen, you’ll see the “Your Schedule” panel. This section helps you stay on top of deadlines with views by day, week, or month. Assignments appear grouped under headings like “Overdue” or “Others,” and if there’s an internal audit due, you’ll see it listed. To get started, just click the “Start” or “Resume” button next.

Before starting an internal audit, you can preview all its contents from the audit overview screen. This includes the audit description, license type, template, and a breakdown of each section and assessment you'll need to complete. Scroll through the audit contents to see each section and its corresponding questions. To begin completing the internal audit, click the **Start Audit button in the top right**. Once launched, you'll see the list of assessments and sections that make up the audit. Select an assessment to begin answering questions within each section.

As you begin answering the questions in your assessment, make sure to read each one carefully and evaluate it against the actual conditions and procedures in your facility. Each question focuses on specific responsibilities or procedures and it’s your job to evaluate whether those standards are being met. You’ll respond to each item by selecting either “Yes” or “No,” depending on whether your facility or the employee you’re evaluating meets the stated criteria.

If you answer “No” to any question, the system will require you to either review and resolve the issue immediately or create a corrective action plan. This ensures that every identified gap is acknowledged and addressed.

If you need more flexibility while answering, click the three-dot action menu to the right of any question. This menu gives you several options: you can reset your answer if you want to change your response, mark the question as not applicable if it doesn’t apply to your facility, mark it as not assessed if you’re unable to evaluate it at the moment, or choose to “Add an Observation”.

When completing your assessment, you may also need to add observations to support your answers. Use the three-dot menu next to any question to open the “Add Observation” window. Here, you can briefly describe what you’ve observed and indicate whether the observation is positive or negative. Positive observations highlight what’s working well, while negative ones point out issues or areas that need attention. If you mark an observation as negative, you’ll see a checkbox labeled “Require Action Plan.” If this box is checked, the system will require you to either review and resolve the issue or create a corrective action plan. If it’s left unchecked, the observation will still appear in the final report but won’t trigger any follow-up action.

To add a note during an assessment, click the Notes icon located below the question. A yellow comment box will appear where you can type your note. Once entered, the note will be saved with a timestamp and the name of the person who added it. You can delete the note by clicking the delete button.

To add photos to a response in an assessment, click the photo icon beneath the question. This opens a panel where you can upload images from your device, such as relevant product or procedure photos. Once uploaded, the image will display below the question.

To attach a file, click the paperclip icon next to the photo option. You can then upload documents like SOPs, or other supporting materials.

Once all assessments in the internal audit have been completed, select **Complete** to submit your audit responses. If you’re not ready to submit yet, choose **Save and Exit** to save your progress and return later. Use **Discard** only if you want to delete the entire audit inputs entirely—this action cannot be undone, so proceed with caution.

After completing an internal audit, you'll see the Results Page which provides a detailed summary of your audit, including your total score, number of points earned, and a comparison to previous results. You’ll also see when and where the audit was completed and who conducted it. At the top, options are available to download the results, share them with others, or generate a certificate for documentation purposes.

You’ll also see any observations or questions that require corrective action plans. Each item has a button that allows you to launch an action plan directly. You’ll also find a breakdown of scores for each assessment area with the ability to review individual sections, responses, and attachments like photos or notes. This page serves as your central hub for reviewing audit findings, identifying areas for improvement, and taking any necessary follow-up actions.

How do I revisit audit results?

From the Internal Audits page, you can easily revisit the results of a completed audit. Locate the “Your Recent Audits” section near the bottom of the screen. Here, you'll find a list of audits you've conducted, along with key details like facility, status, date completed, and audit score. To view more information, click the three-dot menu icon in the “Actions” column. This opens **Result Details**. Select “Result Details” to return to the full results page for that audit.

How do I download the results of my internal audit?

To download your internal audit results, begin by selecting the **Download Result** button from the audit results page. A popup window will appear, giving you control over which sections to include in your download. You can check "All sections" for a comprehensive report or selectively choose individual components like results, previous results, distribution, or any action plans. Once your selections are complete, the **Download** button becomes active. Click to generate and save your customized results report.
