Enter the 'Monitor's Company Name', 'Monitoring Start Date' and then upload the 'Monitoring Document(PDF)'. 

Now the 'Air Monitoring Document' is uploaded and stored here.

You must confirm you would like to delete the document first. Note: This cannot be restored once deleted.
