Monthly Participant Report - Business Planning

This How-To will show you show to complete and upload your Monthly Participant Report. This is the report you will file by the 10th of each month while you're planning your business and working toward completing your business plan.

Click "PDF Document"

Fill in your Agreement ID. This is also called your Contract ID or LAMPS ID and can be found on your ENS Contract.

Click "PDF Document"

In the Reporting period field, enter the last day of the month for the period you're reporting. (Ex: The report you must file by July 10th reports the business plan done in June. The Reporting Period would be: June 30, 202X)

Enter your Business Name. This can be a working title or you can enter "TBD" if it is to be determined later.

Provide a brief description of your main product or service you plan to offer.

Tip! Remember, your ENS Contract requires you work a minimum of 35 hours a week while in the program. Be sure to account for partial weeks at the beginning and end of each month. If only three days of the month fell within reporting week, you're required to report a minimum of 21 hours. Any exceptions should be discussed with your Business Advisor in advance.

Enter a summary of your business planning activities you completed during this reporting week. Include events, projects, research, planning, meetings, etc.

Tip! Remember, you can always reach out to your Business Advisor with questions or concerns. You don't have to wait for your scheduled meeting or the monthly reporting period to ask for help.

Alert! Failure to upload these reports to the Files tab of your CeedWork Program is considered a violation of your ENS Contract and may jeopardize your participation and funding for this program.

Tip! Be sure to include the Reporting Month and Year at the end of the file name.

Click "Add File" and select the PDF you saved.

Click "Drag-and-drop documents here (or click to browse...)"

Click "Open" to begin the upload.

Click " Close"

Tip! To ensure your file successfully uploaded, click on the "Task List" tab of your program, and then click on the "Files" tab again so it will reload. Check to ensure your has been uploaded.
