Moving Clients to Different Tiers; Intake, Assessment, Active, Discharged)

This guide provides essential step-by-step instructions for effectively managing client tiers within a system including; Intake, Assessment, Active, and Discharged stages. By following these procedures, users can ensure clients are appropriately assigned and transitioned throughout the tiers.

Click "Clients"

To move clients out of the Intake Tier, you will navigate to the "Intake" tab.

Click the checkbox next to the client that you would like to move out of intake.

Click "Apply Action"

Click "Assign to...". Choose the Assessor from the list that will be responsible for the client while they are in the Assessment tier. This will move the client to Assessment and provide the assigned assessor access to their chart. **Note: You can also Discharge the client from this space as well if they are not moving forward with services.**

To move clients out of the Assessment Tier, you will navigate to the "Assessment" tab.

Click the checkbox next to the client that you would like to move out of assessment.

Click "Apply Action"

Click "Close Assessment" to move the client to the Active tier. **Note: You can also Discharge the client from this space, or choose to assign them to a different assessor.**

Click "Yes, close assessment" to confirm.

To manage clients in the Active Tier, you will navigate to the "Active" tab.

Click the checkbox next to the client that you would like to manage in Active.

Click "Apply Action"

From here you can use the "Assign QMHPs/Clinicians" option to assign direct staff to this active chart. This allows them to enter into the chart from their "Assigned to Me" area.

Select all staff that should be directly assigned to this client's chart. **Note: If you do not see a staff member here it means that they are not listed as a Direct Care Staff/Clinician in Noteable. They will need this access role for direct assignment.**

Once all applicable staff have been selected, Click "Assign"

To move clients out of the Active tier. Select the checkbox next to their name.

Click "Apply Action"

Click "Discharge" to move the client to the Discharged tier. **Note: You can also move the client back to Intake or Assign for Re-Assessment from this space.**

If you are discharging the client, confirm the date of discharge and select "Yes, discharge" from the pop-out window. This will move the client to the Discharged tab in the Clients area.

Click the checkbox next to the client that you would like to manage in Active.

Click "Apply Action"
