Update your DNS to redirect traffic on subdomain using a CNAME record.
Your CNAME record will look like: ezportal.<Your Request Portal URL>.com IN CNAME milo.ezportal.com

To enable "Non-login Requests", make sure you have "Restricted Access" disabled so non-login members can access your Request Portal.

Enter any Terms and Conditions that must be fulfilled before requesting any item.

Checked-out items must be returned within 1-2 days of the checkout dates. 

Then, you can also enable signatures upon submitting reservation requests and accept any data privacy policies if applicable.
