Navigate to [https://qa-provider.myocchealthrecord.dev/#/app/main/provider/completed-bookings](https://qa-provider.myocchealthrecord.dev/#/app/main/provider/completed-bookings)

Search for the candidate/employee that requires an addendum in the "Completed Bookings" tab.

Next, click the arrow button underneath the actions column and then click add "Add Addendum".

Fill in all required fields to upload the addendum document. This includes the title and document type.

When a document has been uploaded the below message will appear.

Lastly click "Save". The addendum will then appear in the final report.
