Select the form you wish to send from this dropdown

Input the client's name in the "Name" field

Input the client's email in the "Email" field

Input instructions or a personal message in the "Instructions (Optional)" field

\*This will show in the email your client receives

Click "Close" and that's it!  On to the next!

After your client has responded to your form, you will see these responses in the Latest forms received column. 

You can click View and see their responses. 

After reviewing the response form, you can also add an internal note for your lab.  Click "Add Note" and type your note
