After you've selected your contact, click the arrow next to **Edit contact.**

Per CMS compliance, you must have the contact's address before you can send the SOA.

You can send the SOA in both English and Spanish.

Select the Products to discuss and then click "Sign in advance"

When signing in advance, you must follow the 48 hour rule per CMS.

Beneficiaries must sign a Scope of Appointment (SOA) at least 48 hours before a scheduled appointment. For walk-ins, use the sign in person option.

The email address pulls directly from their profile, but you can change it here.

Click "Email appointed representative" if they have an appointed representative like a lawyer.

Click the "\* Appointment date (req.)" field to select the date for the appointment.

You'll see the confirmation pop up after you've sent the SOA.

If sending the SOA directly from Spark, they will receive an automated email and can begin the SOA process.

They'll type in their name after they've added their phone number and click Sign & Continue

Once they've signed, they'll be brought to a confirmation page where they can also download and print their SOA.
