Type in your provider's first and last name

If no existing profiles match your provider, scroll down and click "Create A New Doctor"

Click "Choose a Specialty" and choose the correct specialty from the dropdown menu

Enter the provider's practice name. If no existing practice matches your provider's information, click "Create A New Practice"

When entering the city name, select the correct name from the dropdown menu.

You are done!

The RateMDs team verifies provider information with the respective governing bodies before a provider profile creation can be approved. This process may take up to 3 business days.
