From the landing page, click on Summary Measures to go to the Dashboard 2.0 page.

Welcome to the Summary Measures page, where you will find summary statistics for your practice or PO, including:

- **Outcome measures** like the percentage of patients with *uncontrolled A1C*
- **Screening measures** like the percentage of patients with *retinal exams*
- **Prescribing measures** like the percentage of patients prescribed a *CGM*

View your practice and/or PO's performance on key outcome measures, including.

1. **A1c:** Percentage of patients less than 65 years old with controlled (&lt;8.0%) vs poorly controlled (≥8.0%) within the last 12 months
2. **A1c:** Percentage of patients 65 years old or older with controlled (&lt;8.0%) vs poorly controlled (≥8.0%) within the last 12 months
3. **Blood pressure control:** Percentage of patients with controlled (&lt;140/90) vs poorly controlled ((≥140/90) blood pressure.

Click or hover over on any bar in a chart to see the measure statistic as a pop up box.

In the middle of the page, see two Screening Measures:

- Percentage of patients who have received a Retinal Exam
- Percentage of patients with a reported A1C in the last 12 months.

In the last section of this page, you will find Prescribing Measures. At a glance, find prescribing rates (over the last 12 months) for the following:

- CGM
- SGLT2i
- GLP1 RA
- Insulin
- Sulfonylurea
- Metformin
- Statin (within the last 6 months, stratified by age group)

Click the "Expand" toggle to see a detailed view of each measure.

How to: Drill down to see rates across practices

**Note:** Access to practice- and provider-level comparison may be limited to PO-level users (PO Admins).

Click the "Expand" toggle to expand the view of any measure and see practice- and/or provider- level rates.

You will see a list of practice names with their total number of patients and metric visualized in the bar chart.

Let's zoom in on this example. Listed are the names of your practices: Practice Blue, Practice Green, Practice Red, and Practice Brown.

Practice Blue has 282 patients included in the analysis of patients under 65 with A1c in the last 12 month reporting period. Of those 282, 8.2% have an A1C greater than or equal to 8.0%. 91.8% of those patients have a controlled A1c of less than 8.0%

To the right, you will see a list of patient names and their last reported A1c value.

Click on the Practice Name (or on the Practice's bar) to filter the accompanying patient list to only patients of that practice.

To sort the patient list A-Z by last name: Click on the "Patient name" header.

To return to its default A1c highest to lowest: Click on the

To limit the patient list to only those who do not meet the target, click on the orange area of the bar chart.

In the example below, 27.6% of patients under 65 from Practice Blue haven't met the target. Those 24 patients are listed to the right, with their last recorded A1c in orange.

How to: Drill down to see rates across providers

**PO-level users (PO admins)** can also see a Provider-level breakdown of any measure.

To drill down to the Provider level, hover your cursor over the "Name" heading above your list of practices. You will noticed a very small '+' button (highlighted in orange below. Click to open the provider panel.

You'll now see a panel to the right of practice name that lists the providers at each practice, with the patient count and metrics for each provider.

On the right, the patient list will encompass all practices by default. Clicking on a Provider Name will filter that patient list.

The select Provider Name will be highlighted in blue. You will see the patient list filtered down.

Click on the the blue "Download" button in the bottom right corner to save or print a PDF version of this page.

Under the Scaling drop down, select "At most 1 page wide" to format on a standard printer paper page.
