Step 1.1 Clone the General Webinar Call Opt-in Form

Click on **Guide me** to follow along on **Salesable**!

Log-in to [app.salesble.co](https://app.salesable.co)

On the top menu bar click on **Forms** then **Builder**.

Look for "04. **General Webinar Call Opt-In Form** TEMPLATE ONLY (DO NOT EDIT THIS - CLONE & USE)"

**Edit the Form name** to your preference then Click on **Confirm** to Save.

On the same page Look for "05. **Strategy Call Questions Form** TEMPLATE ONLY(DO NOT EDIT THIS - CLONE & USE)"

Rename it to your preference in the **Form name field**.

Click the **Form name** field then click on **Confirm** to save.

**Congratulations**! You are done cloning your **Calendar form**!

Look for your **Cloned Strategy Call Form**.

**Remove** the **Template questions** in the form.

Click the plus icon on the upper left side to **quick add** questions in the form.

Date of birth

Or you can add your own **Custom Fields** here. 

Click **Add** to create your new Custom Field.

Click **Save** and you're done editing your **Calendar Questions form!**

Then click **Edit the Form name** to your preference on the top menu bar.

Then you can select the **team members** you will assign. Otherwise, you can just select your name.

In the **Custom URL section**, you must set a **new unique name** for the URL booking widget. For example, you can add your name and the purpose of the calendar ie: "John-music-theory"

**Congratulations**! You're done creating your **Calendar Booking**!

Click **Calendar Settings** on the top menu bar.

Scroll down and look for **Zoom** under **Conferencing**.

A new window/tab will open. Then **Sign in** with your **Zoom account information**.
