Customize the Theme of a Google Form

Click the **color palette** icon

The **Theme** panel will open... select your preferred **Text style** options

Under Header, click the **Choose Image** button

You can choose from selected **Themes**, **Upload** a file, or choose from **Photos**

Select your header image... click the **Insert** button

Click the accent and background **Color**...

When you are finished customizing the Theme, click the **\[ X \]**
