Click "Benefit Information" or "Open Enrollment" Tile

Click "2027 BCBS HPN ZIP Code Lookup"

Enter your **Home Zip Code** to see if it is in the HPN Plan Coverage Area

If your Home Address in the HPN Plan service area, you may elect to enroll in this plan.

If your Home Address is outside the HPN Plan service area, please select another plan.

If your Home Address is incorrect or needs to be updated, click the link to update your Contact Information.
