Completing the Academic Integrity Decision Form

Navigate to the Decision Form, located online at <https://cm.maxient.com/reportingform.php?AdelphiUniv&layout_id=31>. Similar to the other reporting forms involved in the academic integrity process, this form provides you with process status information and also asks the reporter to provide their name and email address.

Review the Important Information for Consideration

Just below the background information section, the form provides the reporting faculty member with important information to consider as they generate the decision. Remember that a well-authored decision is more than simply a finding (responsible or not responsible). For a decision to pass muster, both at an internal appeal and through litigation, it must contain supporting information, including a rationale. Please know that the information contained in this form will be incorporated into the final outcome letter sent to the student and also the reporting faculty member. Information contained in this decision form will NOT be edited.

Complete the Six (6) Questions on the Form

The questions on the form provide the Academic Integrity Officer with the information needed to craft the outcome letter. 1. Identify the case number- The case number has nothing to do with the decision, but with multiple academic integrity incidents occurring, this allows the IO to route the form properly. 2. Providing your rationale for the decision is an integral part of the decision. It also helps the IO determine what other educational sanctions should be imposed, if there is a finding of “responsible.” 3. Select the type of meeting held between the faculty and the student. 4. Gathering information about the meeting is helpful to the outcome letter and also will allow the IO to understand the context of academic integrity meetings in a broader context and produce additional resources to aid students and faculty in future meetings. 5. Select the appropriate finding, in this case 6. Identify whether the student accepted responsibility

Include Supporting Documentation and Submit the Form

As with all incident report forms, there is an option to include supporting documentation. This would be unusual at this stage of the process; however, there are situations where this feature may be needed. For instance, a student comes to the meeting with the faculty member and brings their laptop, which contains version histories of the assignment. Or a student shares text messages with the faculty member of the student communicating with another student about the collaboration that occurred. As the faculty member, if you rely on this new information, it is important that you capture that information and submit it so the new information can be contained in the case file. If there is no additional information, then do not concern yourself with that feature, and once you complete the form, press the submit button.

Preparation and Transmission of the Outcome Letter

After the faculty member submits the decision form, the Academic Integrity Officer reviews the information and generates the outcome letter for the student. This outcome letter consists of several parts: 1. The finding(s) 2. The rationale 3. Any imposed sanctions (both academic and educational) The outcome letter is sent to the student electronically, and copies of the outcome letter are sent to the reporting faculty member and the Academic Integrity Advocate. The reporting faculty member must wait until the expiration of the appeal request period (5 business days), and if the student has not submitted an accepted appeal request, the reporting faculty member may then submit the grade. The Academic Integrity Officer will notify the faculty member when it is permissible to enter the grade for the student.
