Step 1

Reporting an academic integrity violation is easy to do. Navigate to the incident reporting page[-https://cm.maxient.com/reportingform.php?AdelphiUniv&layout_id=27](https://cm.maxient.com/reportingform.php?AdelphiUniv&layout_id=27). It is suggested that you bookmark this URL on your computer for ease of reference for future use. This form is available from the “Academic Integrity” tile in your eCampus portal, or from the Student Conduct and Community Standards website. This top section of the form provides information about the academic integrity process. If you have specific questions, please use the contact information on the webpage.

Enter the Background Information

This information includes the following: ● Your name, telephone number, and Adelphi email address\ ● The college or school in which the course report originates. (Select from the dropdown menu)\ ● The academic department in which the course report originates. (Select from the dropdown menu)\ ● The incident date (This should be the date that the assignment/exam was submitted, not the date that the possible misconduct was discovered) The incident reporter can either type in the date or use the calendar icon to select the date.\ ● The location of the incident refers to the platform through which the assignment/exam that contained the possible misconduct was submitted. The building name and room number of where the misconduct occurred is irrelevant.\ All of this information is important; all required information is identified by the word “Required” in red font color.

Alert! Remember that the incident date is the date that the misconduct occurred (e.g. the date the assignment was submitted, or the exam was taken) NOT the date that the misconduct was discovered.

Enter the Involved Parties Information

The first image shows you the fields as it appears on a blank incident report form. The second image shows you the information entered correctly on the form. It is important to include both the accused student (respondent) and the reporting faculty member. If you have any witnesses to the event, please enter their information as well. If no witnesses exist, of course do not concern yourself with that. *Please note that the system asks for your Adelphi University ID number, and the ID number of the student, not your network credentials login. (e.g., network user name)*

Review the Definitions of the Academic Integrity Violations

These definitions have been reviewed by and accepted by the Provost’s Committee on Academic Integrity and the Faculty Senate. When a decision is made about whether a student committed an academic integrity violation, the decision is made by applying the evidence presented to the parameters of these definitions.

Answer the Questions

There are some questions that are required, and other questions that require an answer if the incident reporter marks “Other” in the question above. The questions on this form provide necessary information to share with the student, as well as assist the Academic Integrity Officer with gathering and compiling statistical data regarding academic integrity issues. The questions should be self-explanatory, but we provide a few notes. ● The course name and course number are required and are deliberately split into two (2) different questions.\ ● The possible violations are titled accordingly, and you may select as many violations as you believe evidence exists for.\ ● The description field is a free-text field. Please be as descriptive as possible and provide information that you believe supports any possible violation marked in the above question.\ ● It is important to provide information about the grade that you would issue to the student if they are found responsible for an academic integrity violation, and also the grade you would issue to the student if they are found NOT responsible for the academic integrity violation. The final question about employment status is for statistical purposes only and is not reported to the student.

Tip! When generating your description of the incident, please do not include conclusions or suppositions. Conclusions or suppositions are usually irrelevant and may lead to an erroneous outcome.

Provide Supporting Documentation to Support Your Report

There are a couple of items that are required: ● Your course syllabus\ ● The submitted assignment or exam in question\ ● Anything else that you can use to support your assertion that a violation occurred, including, but not limited to: ○ Copies of Moodle logs\ ○ Screenshots of information located online\ ○ Artificial intelligence prompts and outputs\ ○ Audio and video recordings\ ○ Emails\ It is preferable to submit the attached information as a PDF file, if possible. Please know that all supporting documentation will be shared with the student should they wish to provide a response.\ \ Submit your report. When you finish completing the form and attaching your information, click the blue “Submit” button at the bottom of the form.
