The plan requires a review of the applicant's current health and medical history, meaning the initial quote may change. Depending on this review, applicants might encounter higher premiums, specific benefit exclusions, or a decline in coverage. To apply, customers must complete the online application and submit all required medical history information for each applicant. Our Medical Underwriting team will assess this information to determine the risk and finalize the coverage details.

Getting Started

Before getting started, please confirm that *all* applicants have provincial health coverage and currently have, or have applied for, a Saskatchewan Health Services Card. Click **"Yes"** to confirm and proceed.

To start a new Personal Health Plan application, click **"New Policy"** to begin the application process.

To apply for a Blue Choice Personal Health Plan, 1. Select **"No"**. 2. Choose the **"Blue Choice"** plan to continue with the Blue Choice application. 3. Click **"Health Insurance Plan Comparison"** to view a side-by-side comparison of the available Personal Health Plans.\ \ Click **"Get Started"** to begin the application.

Fill out the required fields for the primary individual applying for coverage. **Note:** This individual will become the Policyholder.

How did the applicant hear about Saskatchewan Blue Cross? Let us know by selecting from the drop-down menu. If the applicant has a promo code, enter it in the **'Referral code'** field. Click **"Continue"** to proceed.

Policyholder Information

Complete the required fields for the Policyholder and click **'Save Applicant'**.

Adding a Partner

Click **'Yes'** to add a Partner to the application.\ \ **Partner:** The partner of the Policyholder by marriage or a cohabitated common-law relationship. Sometimes referred to as a Spouse.

Complete the required fields for the Partner and click **'Save Partner'**.

After clicking '**Save Partner'** a notice will appear, reminding the applicant to complete the medical questions ***ALL*** individuals listed on the application.

Adding a Dependent

Click **'Yes'** to add a dependent child to the application. **Dependent** means a Policyholder or Partner's:\ • unmarried child up to 18 years of age.\ • unmarried child up to 25 years of age who is a Full Time Student.\ • child who due to a physical or mental infirmity cannot leave the care of the Policyholder or Partner.

Click **'Add Dependent'.**

Complete the required fields for the Dependent. If the dependent is between age 18 and 25, a **'Check Box**' will appear to confirm the dependent child is a full-time student, enrolled in full-time courses through an accredited educational institution. Select the **'Check box'** if the dependent is a student. \ \ • If your client advised you their dependent is no longer a full-time student, enrolled in an accredited education institution please email [Brokers@sk.bluecross.ca](mailto:Brokers@sk.bluecross.ca) with this information.\ • If a policy includes dependents age 18 and over, a members renewal notice will remind them to make any changes if eligibility requirements are no longer met.\ • Dependents turning 25 will receive a notification advising they are no longer eligible for coverage under their parent's policy and an offer for a plan of their own.

If the dependent is age 18 or over, a **'Check Box**' will appear to confirm the dependent child is unable to leave the Policyholder or Partner's care due to physical or mental disability. Incapacitated Dependents are eligible to remain on the policy indefinitely. Tick off the **'Check box'** if the dependent is an incapacitated dependent.

Click **'Save Dependent'**

Click **'Add Dependent'** to add additional dependents to the application.\ \ Once all applicants are added to the application, click **'Continue'** to proceed.

Review and select coverage

Review the applicant(s) information. Scroll down to Coverage Options. Each Blue Choice Plan starts with our comprehensive Core Health Benefits. Applicants can customize their plan to suit their needs, budget and phase of life by selecting optional benefits to apply for. Click **'Continue'.**\ \ **Note:** The premium generated is a base-premium quote and may not accurately reflect the final policy premium.

Coordination of Benefits (COB) Details

If any individuals included on the application — including the **policyholder, partner, or dependents** — have **other active health or insurance coverage** (e.g., a Personal Health Plan, employer/group benefits, student coverage, or any other plan): 1. **Select “Yes”** under *Do you have other insurance coverage?* 2. **Click Continue** to proceed. If **no individuals** on the application have other coverage, select **“No”** to proceed.

When you select **“Yes”** to indicate other insurance coverage, a form will generate on screen. 1. **Complete all required fields** using the details from the *employee policyholder’s* insurance plan. 2. **Indicate the type of coverage** included under the other insurance plan. - Select all applicable benefit categories. 3. If any benefits are *not listed* or require clarification, **add them in the Additional Information field** at the bottom of the form. Once all information is entered, **click Save** to continue.

After you save the COB details, the information will be added to the application. 1. If dependents are covered under the other insurance plan, select **Add Dependents** to include them on the COB record. - *Note:* Each COB record allows a maximum of **30 dependents**. This limit is intentionally high to prevent errors for applicants with large families. 2. If the applicant has more than one additional insurance policy, select **Add Another Insurance Plan** to create another COB record. - *Note:* Each application can include up to **20 COB records**. This limit is also intentionally high to avoid processing issues for complex cases. When all COB details have been added, click **Continue** to move to the next step of the application.

Medical Information

Customers who apply for a Blue Choice Personal Health Plan must complete the online application and include requested information related to the medical history of each applicant. Our Medical Underwriting department will then use the medical history to determine if the level of risk to insure the applicants. Based on this assessment, applicants may be offered a plan with the following coverage: - **Clean Application**: One or more of the applicants will be granted full coverage without any adjustments made to the plan. - **Higher Rating**: One or more of the applicants will be charged a higher premium than the base rates to receive coverage for the corresponding benefit. This is usually applied when the applicant has an existing medical condition and poses a greater than average risk. - **Exclusion**: One or more of the applicants may still be given coverage for most benefits but will be excluded from one or more specific benefits. - **Declined**: One or more of the applicants is declined coverage entirely if it is deemed that their medical history poses too great of a risk to insure. If a member of a family is declined, the others may still be eligible for coverage under the policy.

If **'Yes'**, additional information fields will generate and request additional details pertaining to the applicant's medication. Select the **'Ongoing'** checkbox if the medication has no end date and is on-going. Click **'Save'.**

If an error was made, click **'Remove'.** If another applicant is a **'Yes'** to the same statement, select **'Add another record'** and input their information. Select **'Continue'** once ***all*** medication details for ***all*** applicant is input.

Additional Medical History

The Additional Medical History questions are applicable for ***all*** applicants applying for the Blue Choice Plan. Click **'No'** if the statement **does not** apply to any applicants. Click **'Yes'** if a statement applies to ***any*** applicant.

If **'Yes'**, a text box will appear and request additional details pertaining to the applicant's medical history. Click **'Save'.**

If an error was made, click **'Remove'.** If another applicant is a **'Yes'** to the same statement, select **'Add another record'** and input their information.

Proceed through the Additional Medical History questions and select **'Continue'** once ***all*** medical history details for ***all*** applicants is entered.

Agreement and Consent

Review the agreement and consent, in full, with the customer.\ \ Click **'No, I am completing the application on behalf of the applicant (e.g. Brokers, Friends, Family Members)'** and complete the required fields.\ \ Select the **'Check box'** to confirm you and the applicant have read and understand the entire application, agree to the Agreement and Consent and confirm that all questions, for all applicates, are answered to the best of each individual's knowledge and consent. Click **'Continue'**
