On the client's profile page, click the dropdown arrow next to **Edit contact.**

Click **Send needs assessment**.

Check the box next to **Customize needs assessment** if you'd like to customize the information sent to your Prospect.

You'll notice **Health Insurance** and **Veteran Benefits** information is requested for Prospects, but you can easily uncheck those.

Click **Send survey**.

Your Prospect will receive the email to complete the survey.

Your contact will receive an email and can click **Start Form**.

They'll click **Start the survey**, then begin entering their Medicare information.

Once submitted, they'll be brought to a confirmation page and can begin the Scope of Appointment if they'd like. The Prospect will click **Sign Scope of Appointment** to begin the SOA.
