Complete Incident Investigation

Learn how to access and update incident report details within the HammerTech platform. This guide helps you navigate to specific records to modify root cause factors, upload attachments, and save changes for project management reviews.

Click "Incidents" in the sidebar menu, then "View All".

Select the incident record from the register to view its details.

Click "Options" and select "Edit".

Scroll to the **Photos** and **Attachments**. Hit the plus button to add required records to the incident.

Scroll to the **Investigation** sections. Set "Investigation Started" to "Yes" for additional fields to appear. The required details to enter depend on the incident classification (e.g. Basic Investigation, or ICAM investigation)

Complete the investigation form fields.

Under Corrective & Preventative Actions click the blue plus button to raise and assign actions (observations). Note incidents can not be closed out until all Observations (actions) are completed.

For High Potential Incidents (HPIs) add required report details and attachments. Add SMIN, Boardroom Debrief and ICAM Report as required.

Enter the "ROOT CAUSE & CONTRIBUTING FACTORS" details.

Select under the ICAM ANALYSIS section the required codes identified as part of the investigation.

For ICAM Investigations "PROJECT MANAGER REVIEW" to be signed off by Project Manager when completed.

Ensure the "Active (Open)" status is selected (note incident cannot be closed if not in this status).

Click "Save Changes" to finalise the investigation.
