Enter an Incident

As a BMD User enter an incident report. Once entering an incident, email notifications will go to set distribution groups in accordance with BMD Incident Management Standard. Add Injury details as required.

Click "Incidents" from the sidebar menu.

Click "Create New"

Select the Date & Time of the incident then click **Create**.

Enter the "INCIDENT CLASSIFICATION" in accordance with *BMD Incident Management Standard.*

Click 'No Employers Selected' to open the employer selection menu.

Enter the Preliminary Incident Details.

Identify if the incident is a High Potential Incident and or a Breach.

Add photos or attachments by clicking the plus button (eg witness statements, JHA cards, permit to work).

Enter the Notification requirement of the incident, in accordance with *BMD Incident Management Standard.*

Select 'Active (Open)' to finalize the report status.

For the Incident Notification to be sent Scroll to the bottom of the form and **Save Changes.**

Scroll to the bottom of the page. Change status to **Active (Open)**

Alert! Saving changes if ticked as DRAFT will SEND Incident Notification Email to required distribution groups. Do not Save Changes until all details have been entered correctly for distribution.

Select **Save Changes** INCIDENT NOTIFICATION EMAILS WILL BE SENT (IN DRAFT AND ACTIVE STATUS)
