Overview

Teachers will receive an email notification when their supervisor initiates the Formal Observation process. The Formal Observation process has three main phases, including the Pre-Observation, Classroom Observation, and Post-Observation phases.

Teachers who are on the Formal Supervision plan must complete the following items:

Beginning of the Year

• SLO Form 

• Professional Growth Goal

Observation

• Pre-Observation Questionnaire

• Pre-Observation Conference

• Post-Observation Questionnaire

• Self-Assessment Rubric

• Post-Observation Conference

• ​​Observation Summary Form

• Authenticate

End of Year

• SLO Form
