To complete and submit a Threat Assessment, first go to your Outreach dashboard (<https://outreach.ctipractices.com/>) and then select the "Visit Form Filler" button.

This will open the Form Filler page. Navigate to the "Start New Forms and Assessments" screen to begin a new Threat Assessment Instrument. Note: Any forms that are in progress will be listed on this page.

Select the Threat Assessment Instrument to begin. This page is used to start any new assessment or form.

Answer questions and complete the assessment as needed. The assessment will automatically save with any changes that you make. The "Last Modified" date will indicate that an automatic save has taken place.

Search for and select a student to the form via the search bar. There will be identifiable information such as gender, birth date, and age. You will only be able to search for students from within the school to which you have access (or within the school you have selected). If this student has any prior case history, it will be listed directly on this page with a link to view their record in Outreach.

When you have answered all required fields and completed the assessment, the "Submit" button in the upper right of the page will be able to be selected. Note: This button will not be able to be selected if there is any missing required field on the assessment.

If this a new assessment for an incident, you will create a new Concern file to house the assessment and any potential future documents. If this is an additional assessment for a preexisting file, select or search for the preexisting Concern file. In this example, we will create a new Concern file. Enter a title into the "Create a new Concern."

After you have entered your title, click "Submit."

On this page, you can either download the submitted assessment or visit the "Concerns" screen.

On the "Concerns" screen, we see all Concern files that you have access to at your school. Select the title of the Concern you just created to open.

You can view details of the file here. Examples of what you are able to do on this screen: Upload and download any files, view and download any assessments, and view any associated individuals/students.
