How to view any completed or in-progress assessments.

From the home page (<https://outreach.ctipractices.com/>), navigate to the "My Form Submissions" header. To view all, select the "Show All" toggle.

This will list all the forms assessments you have done. Open any by clicking the magnifying glass icon OR the link on the lefthand side of the table.

This will open a link to your assessment. Here you can view the form or print/download your assessment. Selecting the "Print" button will generate a PDF of the assessment.

Selecting the name of the Concern that the assessment is assigned to will open the Concern where you are able to view all related assessments, files, and collaborative efforts for the incident.
