Enter member and animal registration details into the provided fields.

Enter the animal's full tag number into the USDA Scrapie Tag # field.

Select the correct animal color.

Select the correct percentage pureblood.

Enter the animal's date of birth.

Select the animal's sex.

Document Uploads

Upload a copy of the Registration Certificate.

If you have Flock54 results, select 'Yes' for the Flock54 results question.

Click 'Next' to proceed to the next section.

Upload the DNA results spreadsheet from Flock54.

Click 'Next' to continue.

If you do not have Flock54 results for the animal, select 'No' for the Flock54 results prompt.

Click 'Next' to continue.

DNA Kit Selection

Select the DNA kit option you require.

Click 'Next'

Upload DNA results from other laboratories.

Click "Submit" to finish your application.
