How to Lodge a Missing Service Enquiry

Before you start, you will need: - The ABN number of the company you are missing service from - Supporting documents (if available) to support your request. These can include contracts of employment, letters of offer, text messages, emails, payslips etc. - Start and end dates of the missing service period

Click "Lodge a Missing Service Enquiry"

If you have not recently updated your contact details, review and update these before clicking "Lodge Query"

Select the "Missing Service Enquiry" option from the Category dropdown

Select the "Lodge a missing service query" option from the Sub-Category dropdown

Click the "Reason for request" field and add as much detail about your request as possible

If your query is in relation to a current or existing employer who has reported service for you in the past, select them from the dropdown menu. If not, skip this step

If your employer has never reported service for you, click the "My employer is not listed" checkbox

At minimum we require the ABN number of the company you are querying

Populate any other contact details you may have for the company

Confirm if the work was carried out in WA

Confirm if you are currently working for the employer in question

Provide a start date of the period you are querying missing service for

Provide an end date of the period you are querying missing service for

Confirm where the work was carried out

Select your occupation for the missing service in question

Click the declaration checkbox

Upload any supporting document that may support your query. This can include contracts of employment, letters of offer, text messages, emails, payslips.

Click "Submit to MyLeave"
